Introducción: La tomografía de emisión de positrones es una técnica diagnóstica no invasiva que permite tomar imágenes del organismo que muestra el metabolismo de los órganos del cuerpo. Objetivo: Destacar el valor de la PET/CT en el diagnóstico imagenológico prequirúrgico del enfermo. Presentación de caso: Se presentó un paciente masculino de 39 años sin antecedentes de importancia, con un cuadro de hipoglucemias severas de 5 años de evolución, a pesar de los múltiples estudios imagenológicos se incluyó la ecoendoscopía digestiva, lo que no fue posible evidenciar la lesión tumoral. Se le realiza PET/CT cuyo resultado fue crucial para localizar el tumor, se le dio al paciente la oportunidad de un tratamiento quirúrgico y la demostración anatomopatológica de insulinoma. Conclusiones: Los insulinomas son tumores pancreáticos poco frecuentes que provocan hiperinsulinismo endógeno y son difíciles de visualizar debido a su tamaño por las técnicas de imágenes convencionales, por lo que el PET/CT es un estudio bastante efectivo para localizar la lesión tumoral, y así realizar un procedimiento quirúrgico.


Introduction: Positron emission tomography is a non-invasive diagnostic technique, allowing images of the body to be taken that show the metabolism of the body's organs. Objective: To highlight the value of PET/CT in the pre-surgical imaging diagnosis of the patient. Case presentation: We report the case of a 39-year-old male patient with no significant medical history, but a 5-year history of severe hypoglycemia. Despite multiple imaging studies, digestive ultrasound endoscopy was included, which was not possible to demonstrate the tumor lesion. PET/CT was performed, the result of which was crucial in locating the tumor. The patient was given the opportunity for surgical treatment and the pathological demonstration of insulinoma. Conclusions: Insulinomas are rare pancreatic tumors that cause endogenous hyperinsulinism and are difficult to visualize due to their size using conventional imaging techniques, therefore PET/CT is a fairly effective study to locate the tumor lesion, and thus perform a surgical procedure.

RESUMO

Varón de 54 años que consulta por lumbalgia de 5 semanas de evolución refractaria a analgésicos habituales y pérdida de peso significativa. El PET-TAC evidenció una masa retroperitoneal en contacto con la pared anterior de la aorta abdominal. Tras comentarlo con la Unidad de Endoscopias, se decide realizar ecoendosocopia y PAAF por la accesibilidad y el carácter menos invasivo con resultado anatomopatológico de angiosarcoma de aorta. ABSTRACT Background: Endoscopic ultrasonography is used in the diagnosis and treatment of digestive diseases in adults. In children, its use is limited due to a lack of available expertise. Objective: This study aimed to evaluate the clinical impact of endoscopic ultrasonography on diagnostic and therapeutic strategy changes in pediatric patients. Methods: Over ten years, this study retrospectively and consecutively analyzed children aged ≤18 years who underwent endoscopic ultrasonography because of inconclusive imaging or laboratory tests. The indications, results, occurrence of adverse events, and clinical impact of the procedures were analyzed. The clinical impact was classified as major (when the findings led to changes in diagnosis and management), minor (change in diagnosis but not in management), or none (no change in diagnosis or management). Results: Overall, 107 children [77 (72%) of whom were female; mean age: 11.7 ± 4 years] underwent upper [102 (95.3%)] and lower [5 (4.7%)] endoscopic ultrasonography; 64 (58%) patients underwent diagnostic endoscopic ultrasonography, and 43 (42%) underwent interventional endoscopic ultrasonography. Endoscopic ultrasonography was used to investigate pancreaticobiliary, gastric, rectal, esophageal, duodenal, and mediastinal diseases in 81 (76%), 14 (13%), 5 (4.6%), 3 (2.8%), 2 (1.8%), and 2 (1.8%) patients, respectively. The clinical impact was significant in 81% of the children. Major and no clinical impact on pancreaticobiliary, gastrointestinal diseases, and mediastinal masses occurred in 50 (62%) and 13 (16%), 13 (54%) and 9 (37%), and 2 (100%) and 0 (0%) of the patients, respectively. Conclusion: This study evaluated the impact of diagnostic and interventional endoscopic ultrasonography in pediatric patients. When clinically and appropriately indicated, these procedures are safe and effective diagnostic or therapeutic interventions in pediatric patients with gastrointestinal or pancreaticobiliary disorders.


RESUMO Contexto: A ecoendoscopia (EUS) faz parte da prática clínica diária no diagnóstico e tratamento de doenças digestivas em adultos, no entanto, seu uso em crianças é limitado. Objetivo: O objetivo do trabalho foi avaliar o impacto clínico da ecoendoscopia diagnóstica (EUS-D) e ecoendoscopia intervencionista (EUS-I) na população pediátrica. Métodos: Por um período de 10 anos, analisamos retrospectivamente os prontuários de 107 crianças (≤18 anos) submetidas à ecoendoscopia alta [102 (95.3%)] e ecoendoscopia baixa [5 (4.7%)] que tiveram teste de imagem ou laboratorial inconclusivos. O impacto clínico foi classificado como forte (quando mudou o diagnóstico e a terapêutica), fraco (modificou o diagnóstico, mas não o manejo) e ausente (não houve mudança nem do diagnóstico e nem no manejo). Resultados: 107 meninas (72%) e 30 meninos (28%), média de idade 11.7±4 anos (5-18), foram submetidas à ecoendoscopia. 64 (58%) à EUS-D e 43 (42%) à EUS-I [EUS-FNA em 33 (77%) e 10 (33%) a drenagens (pseudocisto (5), walled off necrosis (2), perirectal abscesso (1)) e neurólise do plexo celíaco (2). O sucesso técnico, clínico e a taxa de efeitos adversos para a EUS-I foram de 100%, 90% e 0%, respectivamente. A via biliopancreática foi estudada em 81 (76%), estômago 14 (13%), reto 5 (4.6%), esôfago 3 (2.8%), duodeno 2 (1.8%) e mediastino 2 (1.8%) casos. O impacto clínico total foi de 81%. O impacto clínico foi forte e fraco para a via biliopancreática (81), gastrointestinal (24) e mediastinal (2) em 62% e 16%, 54% e 37% e 100% e 0%, respectivamente. A sensibilidade, especificidade e acurácia da EUS-FNA com microhistologia foi de 76.2%, 100% e 84.8%, respectivamente. Conclusão: Os autores concluem que a EUS-D e a EUS-I são efetivas e seguras quando indicadas corretamente para as doenças digestivas em crianças. A EUS-FNA tem elevada acurácia e pode esclarecer a maioria dos casos duvidosos, determinando o diagnóstico preciso das enfermidades digestivas. O impacto clínico foi grande em relação ao diagnóstico e a mudança do tipo de tratamento na maioria das crianças.

Occasionally, cholecystectomy is not possible because the patient is not suitable for surgery, and non-operative management should be performed. In these patients, the non-operative management can be through the percutaneous transhepatic gallbladder drainage (PTGBD) or the endoscopic gallbladder drainage. We decided to compare the efficacy and safety of PTGBD and EUS-GBD in the non-operative management of patients with acute cholecystitis. We conducted a systematic review in different databases, such as PubMed, OVID, Medline, and Cochrane Databases. This meta-analysis considers studies published until September 2021. Six studies were selected (2 RCTs). These studies included 749 patients. The mean age was 72.81 ±7.41 years, and males represented 57.4%. EUS-GBD technical success was lower than PTGBD (RR, 0.97; 95% CI, 0.95-0.99), whereas clinical success and adverse events rates were similar in both groups. Twenty-one deaths were reported in all six studies. The global mortality rate was 2.80%, without differences in both groups (2.84% and 2.77% in the EUS-GBD group and the PTGBD groups, respectively). EUS-GBD and PTGBD were successful techniques for gallbladder drainage in patients with acute cholecystitis who are non-tributary for surgery. EUS-GBD has a similar clinical success rate and a similar adverse events rate in comparison to PTGBD. The high technical success and the low adverse events rate of the EUS approach to gallbladder make this technique an excellent alternative for patients with acute cholecystitis who cannot be undergoing surgery.


En ocasiones, no es posible realizar una colecistectomía debido a que el paciente no es apto para la cirugía, y se debe optar por un manejo no quirúrgico. En estos pacientes, el manejo no quirúrgico puede ser a través del drenaje transhepático percutáneo de la vesícula o bien el drenaje ecoendoscópico de la misma. En el presente trabajo decidimos comparar la eficacia y seguridad de ambas técnicas en el manejo no quirúrgico de pacientes con colecistitis aguda. Métodos: Se realizó una revisión sistemática en diferentes bases de datos, como PubMed, OVID, Medline y Cochrane Databases. Este metanálisis considera estudios publicados hasta septiembre de 2021. Se seleccionaron seis estudios (2 estudios aleatorizados controlados). Estos estudios incluyeron 749 pacientes. La edad media fue de 72,81 ± 7,41 años, y los varones representaron el 57,4%. El éxito técnico del drenaje ecoendoscópico fue menor que el del drenaje percutáneo (RR, 0,97; IC del 95 %, 0,95-0,99), mientras que las tasas de éxito clínico y de eventos adversos fueron similares en ambos grupos. Se reportaron 21 muertes en los seis estudios. La tasa de mortalidad global fue del 2,80%, sin diferencias en ambos grupos (2,84% y 2,77% en el grupo ecoendoscópico y en el percutáneo, respectivamente). El drenaje ecoendoscópico y el drenaje percutáneo fueron técnicas exitosas para el drenaje de la vesícula biliar en pacientes con colecistitis aguda que no son tributarios de cirugía. El drenaje ecoendoscópico tiene una tasa de éxito clínico similar y una tasa de eventos adversos similar al drenaje percutáneo. El alto éxito técnico y la baja tasa de eventos adversos del abordaje ecoendoscópico de la vesícula biliar hacen de esta técnica una excelente alternativa para pacientes con colecistitis aguda que no pueden ser intervenidos quirúrgicamente.

Antecedentes y objetivo: la ecoendoscopia con punción-aspiración con aguja fina (USE-PAAF) es el método de elección para la toma de muestras de lesiones sólidas pancreáticas. Sin embargo, existe una heterogeneidad significativa en cuanto a la técnica utilizada. La aspiración intermitente no ha sido evaluada en lesiones sólidas pancreáticas y podría mejorar el rendimiento diagnóstico.Métodos: estudio piloto de no inferioridad, simple ciego. Se incluyeron de forma prospectiva pacientes con lesiones sólidas pancreáticas e indicación de USE-PAAF. Los pacientes fueron asignados aleatoriamente para el método de aspiración intermitente (AI) o continua (AC). Se evaluaron el rendimiento diagnóstico, la celularidad, la contaminación hemática y el número de pases necesarios para llegar al diagnóstico.Resultados: treinta y tres pacientes fueron asignados a AC (16 pacientes) o AI (17 pacientes). El rendimiento diagnóstico fue del 87,5 % en AC y del 94,1 % en AI (OR 2,29, IC 95 %: 0,19-27,99, p = 0,51). En el grupo de AI las muestras tuvieron mayor celularidad (OR 1,83, IC 95 %: 0,48-6,91, p = 0,37) y menor contaminación hemática (OR 0,38, IC 95 %: 0,09-1,54, p = 0,18). El número de pases necesarios para llegar al diagnóstico fue de 2,12 en AC y 1,94 con AI (p = 0,64). Se obtuvo citología líquida en el 73,3 % de AI y en el 61,5 % de AC (OR 1,72, IC 95 % 0,35-8,50).Conclusiones: la técnica de AI no fue inferior a la AC en términos de precisión diagnóstica, con tendencia a obtener mayor celularidad, menor contaminación hemática y frecuente presencia de bloque celular en la evaluación de lesiones sólidas pancreáticas. This greater sensitivity, especially in the diagnosis of microlithiasis/biliary sludge, facilitates the indication of cholecystectomy in patients with symptoms of probable biliary origin but may result in over-indication of this surgery.Objectives: Evaluate the role of EUS in the diagnosis of minilithiasis/biliary sludge in patients with digestive symptoms of probable biliary origin by resolving the symptoms after cholecystectomy. Analyse factors related to the remission of symptoms following cholecystectomy.Patients and methods: Retrospective, longitudinal, single-centre study based on a prospective database of 1.121 patients undergoing EUS. Seventy-four patients were identified as meeting inclusion-exclusion criteria (diagnosed with minilithiasis/sludge by EUS after presenting digestive symptoms of probable biliary origin without a history of complicated cholelithiasis). A telephone questionnaire for symptoms was conducted with cholecystectomized patients. Factors related to a good response were analysed with logistic regression analysis.Results: Of the 74 patients, 50 were cholecystectomized (67.5%), mean age 49 years (SEM 2.26) (41 women). Seventy percent of patients (35/50) presented remission of symptoms with median follow-up 353.5 days (95% CI, 270–632.2). The only variable associated with remission of symptoms was the presence of typical biliary colic with an OR of 7.8 (95% CI, 1.8–34; p=0.006). No complications associated with EUS were recorded. One patient (2%) suffered haemoperitoneum and 18% (9/50) suffered diarrhoea following cholecystectomy.Conclusions: EUS is a very useful technique for the indication of cholecystectomy in patients with minilithiasis/sludge and typical symptoms of biliary colic.


Introducción: La ecoendoscopia (USE) presenta mayor sensibilidad que la ecografía abdominal para el diagnóstico de litiasis biliar. Esta mayor sensibilidad, sobre todo en diagnóstico de microlitiasis/barro biliar, facilitaría una colecistectomía en pacientes con clínica digestiva de probable origen biliar, aunque podría inducir su sobre indicación.ObjetivosEvaluar el papel de la USE en el diagnóstico de minilitiasis-barro biliar en pacientes con clínica digestiva de probable origen biliar y ecografía normal mediante la resolución de los síntomas después de colecistectomía. Analizar los factores relacionados con la remisión de la sintomatología tras colecistectomía.Pacientes y método: Estudio retrospectivo, longitudinal, en un centro, sobre base de datos prospectiva de 1.121 pacientes sometidos a USE. Setenta y cuatro pacientes cumplían criterios inclusión-exclusión (realización USE para estudio de minilitiasis/barro por síntomas digestivos de probable origen biliar, sin historia de colelitiasis complicada). Se realizó cuestionario telefónico de síntomas a los pacientes colecistectomizados. Los factores relacionados con buena evolución se analizaron con análisis de regresión logística.Resultados: De los 74 pacientes, 50 fueron colecistectomizados (67,5%), edad media 49 años (ESM 2,26) (41 mujeres). El 70% de pacientes (35/50) presentó remisión de la sintomatología (mediana de seguimiento 353,5 días) (IC 95%, 270-632,2). La única variable asociada con una remisión de la sintomatología fue la presencia de cólico biliar típico con una OR de 7,8 (IC 95%, 1,8-34; p=0,006). No se registró ninguna complicación asociada a la USE. Un paciente (2%) sufrió hemoperitoneo y un 18% (9/50) presentaron diarrea post-colecistectomía.Conclusiones: La USE es de gran utilidad para la indicación de colecistectomía en los pacientes con minilitiasis-barro biliar y clínica típica de cólico biliar.

RESUMEN Antecedentes: tanto el adenocarcinoma gástrico (ACG) como el esofágico (ACE) son una de las princi pales causas de muerte por cáncer digestivo en el mundo, si bien la ecoendoscopia (EUS) ha demos trado ser una herramienta valiosa en la estadificación preoperatoria del ACG y ACE en casos seleccio nados. Objetivo: evaluar la utilidad de la EUS en la estadificación de ACG y ACE para seleccionar los pacientes candidatos a neoadyuvancia, comparándola con la etapa previa a la implementación de la EUS en un centro quirúrgico de la Argentina. Material y métodos: se incluyó una serie consecutiva de pacientes con ACE y ACG durante el período 2013- 2019. Se excluyeron pacientes con criterios de irresecabilidad y operados de urgencia. Se dividió la muestra en cuatro grupos: G1 y G2 ACE con y sin EUS, G3 y G4 ACG con y sin EUS, respectivamente. Se evaluaron variables clínicas, anatomopatológicas y de supervivencia en todos los grupos. Resultados: se incluyó un total de 89 pacientes, de los cuales 40 fueron por ACE, 30 pacientes perte necieron a G1 vs. 10 a G2. Se analizaron 49 pacientes con ACG, 20 pertenecieron a G3, mientras que 29 a G4. En los pacientes estadificados con EUS, en G1, 23 pacientes realizaron neoadyuvancia (76 %) versus 2 pacientes en G2 (20 %) p: ≤ 0,005. En G3 realizaron quimioterapia perioperatoria 8 pacientes (40 %), mientras que en G4, solo dos pacientes (7 %) p: ≤ 0,005. En cuanto al análisis de metástasis ganglionares (G+) en la anatomía patológica, tuvimos un aumento esperable en los pacientes no esta dificados con EUS en cáncer esofágico donde, en G1, el 30% tuvieron G+ versus 60% en G2 p: ≤ 0,005. En G3 y G4 se observó un 45 % de G+. Con un tiempo promedio de seguimiento de 36 meses (6-72), si bien no observamos una diferencia significativa en cuanto a supervivencia global ni recurrencia, observamos una tendencia a favor de los pacientes estadificados con EUS. Conclusión: la utilización de la EUS en la estadificación preoperatoria de ACG y ACE es importante. Aunque su uso puede ser un desafío en muchos centros de la Argentina, futuros esfuerzos son necesa rios para incluir) este estudio en casos seleccionados en la estadificación de tales pacientes.


ABSTRACT Background: Gastric adenocarcinoma (GAC) and esophageal adenocarcinoma (EAC) are one of the leading causes of mortality from gastrointestinal cancer worldwide. Endoscopic ultrasound (EUS) has proved to be a valuable tool for preoperative staging of GAC and EAC in selected cases. Objective: The aim of this study was to evaluate the usefulness of EUS for staging of EAC and GAC and selecting patients who are candidates for neoadjuvant therapy, as compared with the previous stage before the implementation of EUS, in a surgical center in Argentina. Material and methods: Consecutive patients with EAC and GAC between 2013-2019 were included. Patients with criteria of unresectable cancer or who underwent emergency surgery were excluded. The sample was divided into four groups G1 and G2 (EAC with and without EUS, respectively) and G3 and G4 (GAC with and without EUS, respectively). The clinical and anatomopathological variables and survival were evaluated in all the groups. Results: A total of 89 patients were included, 40 with EAC (30 in G1 and 10 in G2, and 49 with GAC, 20 in G3 and 29 in G4. Of the patients undergoing EUS staging in G1, 23 (75%) received neoadjuvant therapy vs. 2 patients in G2 (20%) (P ≤ 0.005). Eight patients (40%) in G3 and 2 (7%) in G4 received perioperative chemotherapy (P ≤ 0.005). Lymph node metastases were observed in 9 (30%) of surgical specimens of EAC in G1 and in 60% in G2 (P ≤ 0.005), and in 45% in G3 and G4. After a mean follow-up of 36 months (6-72), we observed a non-significant trend toward higher overall survival and disease-free survival in patients undergoing EUS staging. Conclusion: EUS for preoperative staging pf EAC and GAC is a useful tool. Although the use of EUS use may be a challenging task in many centers in Argentina, future efforts are needed to include this test in selected cases for staging patients with these types of cancers.

El cáncer de pulmón es la principal causa de muerte por cáncer en todo el mundo. Los nódulos pulmonares ubicados en proximidad al mediastino, retrocardíacos, cercanos a grandes vasos o por delante de la columna vertebral pueden resultar de difícil acceso por vía percutánea o broncoscópica. La punción aspiración/biopsia con aguja fina guiada por ecoendoscopía transesofágica (EUS-FNA/FNB) es un método mini invasivo con baja morbilidad que permitiría acceder a estas localizaciones. Presentamos el caso de un paciente con nódulo pulmonar solitario, en el que se obtuvo el diagnóstico de cáncer de pulmón mediante EUS-FNA/FNB.


Lung cancer is one of the leading causes of death worldwide. Pulmonary nodules located in the vicinity of the mediastinum, retrocardiac, near the aorta or pulmonary vessels, and in front of the spine, may be difficult to access through a percutaneous or bronchoscopic approach. Fine needle aspiration/biopsy guided by transesophageal echoendoscopy (EUS-FNA/FNB) is a minimally invasive method with low morbidity that could allow access to lesions in these places. We present the case of a patient with a solitary pulmonary nodule, in which the diagnosis of lung cancer was obtained by EUS-FNA/FNB.

RESUMEN El empleo del contraste endovenoso permite aumentar la capacidad diagnostica de la ecoendoscopía en muchas condiciones patológicas de forma no invasiva, mediante la evaluación de patrones de realce en tiempo real (hipo, iso e hipercaptación), permitiendo a su vez definir diagnósticos diferenciales o predecir diagnósticos histológicos con alta precisión por medio de la caracterización de la vascularidad o microvasculatura de un órgano o lesión. Sin embargo, su empleo en nuestro medio está limitado por no contar con estos medios de contraste. Reportamos tres casos de ecoendoscopía contrastada realizada en patología pancreática realizada en nuestro hospital, en el que se describe la utilidad en el diagnóstico y manejo de lesiones sólidas y quísticas del páncreas.


ABSTRACT The intravenous contrast (IV) allows for improving the diagnostic accuracy of echoendoscopy for many pathologic conditions. IV contrast provides a non-invasive method that through the use of real-time enhancement patterns (hypo, iso, and hypercaptation), a highly accurate histological diagnosis can be made by characterizing the microvasculature of an organ or a lesion. However, the lack of availability of IV contrast limited its use in our setting. We reported three cases of endoscopic ultrasound with IV contrast performed in the pancreatic department of National Hospital Eduardo Rebagliati Martins in Lima, Peru. We described the effectiveness of IV contrast in the diagnosis and the management of solid and cystic lesions in the pancreas.

Resumen Objetivos: presentar el desarrollo de modelos educativos para el aprendizaje de dos técnicas endoscópicas vigentes, ampliamente difundidas y de gran implicación clínica, con la única intención de permitir la adquisición de destrezas básicas y avanzadas a residentes y gastroenterólogos. Materiales y métodos: se idearon dos modelos sencillos, de muy bajo costo, fácilmente reproducibles y reutilizables, con los cuales se logra ejecutar la totalidad de los procedimientos descritos. Además, permiten al operador integrar el desarrollo de habilidades y la adquisición de los conceptos teóricos mínimos requeridos, sin las presiones generadas por el riesgo existente de complicaciones. Resultados: la tendencia actual a nivel mundial se conduce hacia el desarrollo de modelos de enseñanza que aceleren la curva de aprendizaje, así como de procedimientos altamente exigentes y asociados a complicaciones potencialmente graves. Con estos modelos es posible poner a prueba al endoscopista, mediante una evaluación continua y supervisada. Su implementación en unidades de gastroenterología es sencilla, sin la necesidad de una inversión superlativa o el desplazamiento a otros países. Conclusiones: se trata de un gran aporte al desarrollo científico y educativo de nuestro país, ya que la creación y la implementación de nuevas técnicas endoscópicas y su aprendizaje no deben ser asumidos por los pacientes. En este punto, estamos de acuerdo con los conceptos emitidos por diferentes asociaciones médicas respecto a que los cursos teórico-prácticos de corta duración -y en algunas ocasiones virtuales- no constituyen una formación mínima, requerida para lograr la acreditación.


Abstract Objectives: This article presents the development of educational models for learning two widespread recent endoscopic techniques which have great clinical implications. Its sole intention is to allow acquisition of basic and advanced skills by residents and gastroenterologists. Materials and methods: Two simple, very low cost, easily reproducible and reusable models were devised. Procedures are fully described in ways that allow the operator to integrate the development of skills and acquisition of the minimum theoretical concepts required without the pressures generated by risks of complications. Results: The current global trend is to develop teaching models that accelerate the learning curve for highly demanding procedures that are associated with potentially serious complications. With these models it is possible to test endoscopists through continuous supervised evaluations. Implementation by gastroenterology units can be done easily without the need for large investments or travel to other countries. Conclusions: This is a great contribution to the scientific and educational development of Colombia since neither development of new endoscopic techniques nor the process of learning how to perform them should put patients at risk. We agree with the ideas of numerous medical associations regarding theoretical-practical courses of short duration even though some virtual sessions, "do not constitute the minimum training required needed for accreditation".

Resumen La dispepsia se define como un dolor o molestia abdominal superior que se considera originado en el tracto gastrointestinal superior. Muchas enfermedades y condiciones clínicas pueden producir dispepsia como la úlcera péptica, cáncer gástrico o esofágico, medicamentos, litiasis biliar, pancreatitis, cáncer de páncreas, entre otras. Por una parte, tradicionalmente la dispepsia solo se evalúa con la endoscopia digestiva y su producción solo es del 27 %. Por otra parte, la ecoendoscopia (ECE), al combinar una imagen endoscópica y una imagen ecográfica, podría ampliar el rango diagnóstico al detectar más causas de dispepsia que puedan ayudar a tratar al paciente de manera más oportuna. Objetivo: evaluar si la ECE aumenta el rendimiento diagnóstico que tiene la endoscopia (27 % en nuestro medio) en el enfoque inicial de la dispepsia no investigada (DPNI). Materiales y métodos: estudio prospectivo, de prevalencia analítica en pacientes adultos con DPNI, que consultaron a una institución universitaria de Colombia. Los pacientes incluidos consultaron a la unidad de gastroenterología durante el período comprendido entre enero a octubre del 2016 y se programaron para endoscopia digestiva alta (EDA). Bajo sedación guiada por anestesiólogo primero se realizó la fase endoscópica evaluando el esófago, estómago y duodeno; después se realizó la fase ecográfica y se inició el examen de manera retrógrada evaluándose el páncreas en su totalidad, la vía biliar extrahepática, la vesícula biliar, el tronco celíaco, el lóbulo izquierdo del hígado y la región mediastinal. Todas las anormalidades se anotaban en el formulario de ingreso del paciente. Resultados: En total se incluyeron a 60 pacientes, de los cuales el 65 % son de sexo femenino con un promedio de edad de 40,8 (desviación estándar: 12,5). Los hallazgos en la fase endoscópica de la ECE fueron principalmente gastritis crónica en 43 pacientes (71,6 %), el resto tenía una lesión estructural (17 pacientes): esofagitis en 5 (8,3 %), úlcera gástrica en 2 (3,3 %), úlcera duodenal en 5 (8,3 %), cáncer gástrico en 4 (6,6 %) y lesión subepitelial gástrica (GIST) en 1 (1,6 %). Los hallazgos en la fase ecográfica de la ECE fueron colelitiasis en 11 (18,3 %), coledocolitiasis en 1 (1,6 %) y pancreatitis crónica en 5 (8,3 %). Aunque solo el 28,3 % (17) de los pacientes con dispepsia tuvo un hallazgo estructural en la fase endoscópica, 18 pacientes (30 %) presentaron algún hallazgo positivo en la fase ecográfica, es decir que el rendimiento subió a un 58,3 % (p <0,001). Conclusión: este estudio, aunque pequeño, sugiere que la ECE como método inicial de enfoque en la evaluación de la dispepsia podría llegar a ser útil dado que aumenta el rendimiento diagnóstico en este grupo de pacientes, pasando de un 28,3 % a 58,3 %, lo cual es muy significativo si se tiene en cuenta que estos pacientes con dispepsia y endoscopia negativa usualmente se catalogan como funcionales y solo se les formula medicamentos. Sin embargo, se deben reconocer las fallas metodológicas del estudio y consideramos que es una exploración inicial y deben plantearse estudios más grandes y controlados para confirmar este trabajo; además, se debe tener en cuenta el costo del examen, que es mucho mayor que el de la EDA.


Abstract Dyspepsia is defined as upper abdominal pain or discomfort that is considered to originate in the upper gastrointestinal tract. Many diseases and clinical conditions can cause dyspepsia. Among others, they include peptic ulcers, gastric and esophageal cancer, medications, biliary lithiasis, pancreatitis, and pancreatic cancer. Traditionally, dyspepsia is only evaluated with digestive endoscopy whose diagnostic yield is only 27%. On the other hand, endoscopic ultrasound combines an endoscopic image and an ultrasound image thereby potentially broadening diagnostic range to detect more of the causes of dyspepsia allowing treatment of patients in a timelier manner. Objective: To evaluate whether endoscopic ultrasound increases the diagnostic yield of endoscopy (27% in our environment) in the initial approach to previously unstudied dyspepsia. Materials and methods: This is a prospective study of analytical prevalence in adult patients with previously unstudied dyspepsia who were examined at a university institution in Colombia. The patients included were seen in the gastroenterology unit from January to October 2016 and underwent upper digestive endoscopy and endoscopic ultrasound. Under anesthesiologist-guided sedation, the stomach and duodenal esophagus were first evaluated endoscopically. Then retrograde endoscopic ultrasound was used to evaluate the pancreas in its entirety, the extra hepatic bile duct, the gallbladder, the celiac trunk, the left lobe of the liver and the mediastinal region. All abnormalities were noted on the patient's admission form. Results: In total we included 60 patients of whom 65% were female and whose average age of was 40.8 years (SD: 12.5). The findings in the endoscopic phase of the endoscopic ultrasound were mainly chronic Gastritis 43 patients (71.6%), the rest had a structural lesion (17 patients): esophagitis 5 (8.3%), gastric ulcer 2 (3.3%), duodenal ulcer 5 (8.3%), gastric cancer, 4 (6.6%), gastric subepithelial lesion (GIST) 1 (1.6%). In the endoscopy phase, we found 11 cases of cholelithiasis (18.3%), one case of choledocholithiasis (1.6%), and five cases of chronic pancreatitis (8.3%). Only 17 patients of these patients (28.3%) had a structural finding in the endoscopy phase, but 18 additional patients (30%) had some positive finding in the ultrasound phase. In other words, the diagnostic yield rose to 58.3% (p < 0.001). Conclusion: Although this study's sample size is small, it suggests that using endoscopic ultrasound in the initial evaluation of dyspepsia could be useful since it increased diagnostic yield in this group of patients from 28.3 to 58.3%. This is very significant because patients with dyspepsia and negative endoscopy are usually classified as functional and only treated with medications. However, in recognition of the methodological limitations of this study, it should be considered an initial exploration. Larger, controlled studies should be considered to confirm this work. Another factor that should be considered is the cost of endoscopic ultrasound which is much higher than the upper digestive endoscopy.

Resumen Se presenta el caso clínico de un paciente masculino de 74 años quien ingresó con síntomas de sangrado digestivo alto, se realizó una endoscopia de vías digestivas altas y se encontraron dos lesiones en el estómago: una elevada de centro ulcerado y otra subepitelial, lo que llevó a completar los estudios con una ecoendoscopia gástrica que mostró una lesión mucosa que infiltraba la submucosa sugestiva de cáncer gástrico temprano y otra lesión subepitelial dependiente de la muscular, sugestivas de un tumor gastrointestinal estromal (GIST); los estudios de estatificación no evidenciaron compromiso metastásico, por lo que se llevó al manejo quirúrgico con la posterior confirmación histológica de los hallazgos.


Abstract We present the case of a 74-year-old male patient who was admitted with symptoms of upper digestive bleeding. Endoscopy of his upper digestive tract found an ulcerated lesion and a subepithelial lesion in his stomach. Complete studies including gastric endoscopic ultrasound showed a mucosal lesion infiltrating the submucosa which was suggestive of early gastric cancer as well as a subepithelial lesion on the muscle that was suggestive of a gastrointestinal stromal tumor (GIST). Staging showed no metastatic compromise, so surgery was performed, and histology subsequently confirmed the findings.

Acute pancreatitis is an entity of notable importance due to its high incidence and its non-negligible morbidity and mortality. Idiopathic acute pancreatitis is that in which the cause of the clinical condition cannot be determined after an initial basic study. Understanding the underlying aetiology enables clinicians to propose a targeted treatment to reduce the risk of recurrence. Endoscopic ultrasonography and magnetic resonance cholangiopancreatography are the tests of choice to deepen the aetiological study. The main cause is undiagnosed lithiasic disease in the initial study, whose treatment of choice is cholecystectomy. Moreover, recurrent acute pancreatitis is diagnosed after 2 or more episodes of acute pancreatitis. The objective of this review is to provide an updated approach for these 2 entities, reviewing aspects of their epidemiology, diagnosis and available alternative therapies.

Resumen Los quistes pancreáticos comprenden una amplia variedad de lesiones cada vez más frecuentemente diagnosticadas debido tanto al empleo creciente de técnicas de imagen como al envejecimiento de la población. Entre ellas, las neoplasias quísticas mucinosas son especialmente relevantes por su potencial de malignización. Aunque la ecografía abdominal, la tomografía axial computarizada y la resonancia magnética suelen ser las pruebas diagnósticas de imagen iniciales, muchas veces los hallazgos morfológicos no son suficientes para su diferenciación. La ecoendoscopia se ha convertido en la mejor prueba para su caracterización ya que permite realizar estudio morfológico y también del líquido obtenido mediante punción, aunque su precisión diagnóstica para la detección de quistes de estirpe mucinosa y de malignidad sigue siendo baja. La importancia de la adecuada caracterización radica tanto en la detección precoz de las lesiones preneoplásicas y malignas como en evitar cirugías innecesarias. La indicación de ecoendoscopia, de tratamiento quirúrgico y de seguimiento varía entre las distintas guías de práctica clínica estando actualmente en duda especialmente el tratamiento y seguimiento de las neoplasias quísticas mucinosas papilares intraductales de rama lateral por el menor riesgo de degeneración y su asociación con el cáncer de páncreas.


Abstract Cystic pancreatic lesions comprise a wide variety of lesions that are being increasingly diagnosed due to the more frequent use of imaging techniques and the aging of the population. Among these lesions, mucinous cystic neoplasms are especially relevant because of their malignant potential. Although abdominal ultrasound, computerized tomography and magnetic resonance imaging are usually the initial diagnostic imaging tests, morphological findings are often not enough for their differentiation. Endoscopic ultrasound has become the best test for their characterization because it allows morphological study and fluid analysis obtained by puncture of the lesion, although its diagnostic accuracy for the detection of mucinous and malignant cysts remains low. The importance of proper characterization is the early detection of preneoplastic as well as malignant lesions and to avoid unnecessary surgery. Clinical practice guidelines differ about the indications for endoscopic ultrasound, surgical treatment and follow-up of these lesions. Questions specially remains in the management of side-branch intraductal papillary neoplasm because of their lower risk of degeneration and their association with pancreatic cancer.

The growing incidence of cystic pancreatic tumours has become a major problem in daily clinical practice. These patients usually undergo follow-up programmes of questionable clinical efficacy that put significant strain on endoscopy units. Safe and effective alternatives to surgery are desperately needed in these cases. The aim of this study was to critically review the utility of ablative therapies in cystic pancreatic tumours.

INTRODUCTION AND AIMS: Neuroendocrine tumors are of great scientific interest, given that they are difficult to diagnose and treat. Despite being relatively rare (< 1/100,000 individuals, 1-2% of the gastrointestinal neoplasias) and indolent, their potential malignancy must not be forgotten. An increase in the number of diagnosed tumors has been observed in recent years. The aim of the present study was to update a published case series of 19 patients suspected of presenting with pancreatic neuroendocrine tumor with 51 current cases, to study and compare the new results with those of the previous case series, as well as with other recent publications from Spain, the United States, China, and India. MATERIALS AND METHODS: A retrospective, multicenter case series was conducted on 70 patients (19 cases published in 2011), whose data has been collected over a period of 23 years. The variables analyzed were: age, sex, symptomatology, tumor size, location, metastasis, final diagnosis, and surgery, among others. RESULTS: Mean patient age was 55 years and 60% of the patients were men. Disease location was the pancreatic head in 28.5% of the patients and the tail in 27.1%, mean tumor size was 3.9cm (0.2-10cm), 71.4% of the patients had non-functioning tumors, 32.8% had metastases (100% to the liver), 74.2% of the patients were operated on, and actuarial survival was 75%. CONCLUSIONS: Differences were observed between the previously published case series and the current results. There was an increase in incidentalomas and non-functioning tumors, but no variation in the overall survival rate. The differences with other case series (age, sex, and tumor location) were dependent on the country where the cases were compiled. The increase in tumors could be related to a higher number of diagnoses made through imaging studies and to the greater sensitivity of the devices employed.

Background: Endoscopic ultrasound training has a learning curve greater than the other endoscopic therapeutic techniques. One of the preclinical teaching methods is the use of ex vivo porcine models. Aim: To describe five ex vivo porcine models for training in therapeutic echoendoscopic procedures. Materials and methods: Using porcine digestive tract containing esophagus, stomach, duodenum, spleen, liver and gallbladder, five models for therapeutic echoendoscopy training were described. With other segments of the porcine pieces (such as the bladder, spleen segment and omentum segment) and with easily accessible materials (such as grape and ultrasound gel), lesions were simulated to be treated. These models were applied in the Hands on course at the IRCAD (Institut de recherche contre les cancers de l'appareil digestif) Barretos of 2017. Endoscopic equipment and instruments are the same as those used in clinical practice. Result: The models are easily reproducible and do not require exchange during the hands on course period. Endoscopic and echographic imaging and tactile sensitivity are similar to the real one. Conclusion: The models described in this study demonstrated to be realistic, easy to reproduce and allow repetition during the same session. However, comparative studies are necessary to verify the real impact on teaching


Racional: El entrenamiento de la ecoendoscopía tiene una curva de aprendizaje mayor que las demás técnicas endoscópicas terapéuticas. Uno de los métodos de enseñanza preclínica es el uso de modelos porcinosex vivos. Objetivo: Describir cinco modelos porcino sex vivo para entrenamiento de procedimientos ecoendoscópicos terapéuticos. Materiales y método: Utilizando el tracto digestivo porcino, que contiene esófago, estómago, duodeno, delgado, hígado y vesícula biliar, se han descrito cinco modelos para el entrenamiento de ecoendoscopía terapéutica. Con otros segmentos de la pieza porcina (como vejiga, segmento de delgado, bazo y omento) y con materiales de fácil acceso (como uva y gel de ecografía), se simularon lesiones a ser tratadas. Estos modelos se aplicaron en el curso Handsonenel IRCAD (Institut de recherche contre les cancers de l'appareil digestif) Barretos de 2017. Los aparatos e instrumentos endoscópicos son los mismos utilizados en la práctica clínica. Resultado: Los modelos forman de fácil reproducibilidad, no siendo necesario el cambio de la pieza porcina durante el período del curso Handson. La imagen endoscópica y ecográfica y la sensibilidad táctil son similares a la real. Conclusión: Los modelos descritos en este trabajo han demostrado ser realistas, de fácil reproducción y permiten repetición durante la misma sesión. Sin embargo, los estudios comparativos son necesarios para verificar el impacto real en la enseñanza

BACKGROUND: Most pancreatic cysts (PCs) found incidentally by CT and MRI scans might not be clinically important according to the Fukuoka guidelines, the American Gastroenterological Association (AGA) guidelines and European guidelines. AIMS: To determine and compare the prevalence of incidental clinically important PCs (CIPCs). METHODS: Abdominal contrast-enhanced CT or MRI scans performed during a one-year period were retrospectively reviewed to identify incidental PCs. CIPCs were defined as those cysts that would be capable of triggering further evaluation with endoscopic ultrasound, immediate surveillance (within 3-6 months) and/or surgery. Prevalence was calculated as the number of patients with CIPCs per 100 subjects imaged (%). RESULTS: Sixty patients (mean age 70±14 years) out of 565 were found to have incidental PCs, representing a prevalence of 8.7% (95% CI 6.3-11.5) in CT scans and 27.5% (95% CI 16-41) in MRI scans. Seven patients (11.6%, 95% CI 5-22) had CIPCs based on size ≥ 30mm (n=5), size ≥ 30mm and pancreatic duct (PD) dilation (n=1) and PD dilation and presence of solid component (n=1). Based on the Fukuoka guidelines, the prevalence of CIPCs was 1.2% (95% CI 0.4-2.5) in CT scans (6/507) and 1.7% (95% CI 0.1-9) in MRI scans (1/58). Based on the AGA and European guidelines, the prevalence of CIPCs was 0.2% (95% CI 0.1-1) in CT scans (1/507) and 1.7% (95% CI 0.1-9) in MRI scans (1/58). Patients with PCs initially classified as "AGA- or European-positive" had a higher surgical probability and this decision was taken earlier in the follow-up. CONCLUSIONS: In our cohort, the prevalence of important incidental pancreatic cysts was not negligible at around 1% according to current guidelines.
